  • NWAFT: Formulary

  • CUHFT: Prescribed by Pyschiatrists only.

  • RPH: For patients with poor dietary intake. Specific groups to be defined. Use capsules in preference, keep soluble tablets for patients with swallowing difficulties or for nasogastric tube administration. 

  • Non formulary in Primary Care: Multivitamins can be purchased from a Community Pharmacist.

  • CPFT: Restricted to patient with an eating disorder and who have swallowing difficulties.
  • CUHFT: Restricted to adult critical care patients with limited oral intake requiring vitamin and trace element supplementation whilst on parenteral nutrition [Short term use].
  • Non formulary in Primary Care: Multivitamins can be purchased from a Community Pharmacist.

Dietary supplement, listed as a borderline substance in the drug tariff, therefore requires ACBS endorsement on an FP10 prescription if using for "the dietary management of water soluble vitamin deficiency in renal failure patients receiving dialysis"
